Once you receive this brochure, the instructions will guide you with the payment methods and the best and fastest way to send the following documents needed to obtain the license:

Application form (attached) with your information and signature
Color photo with blank background (passport photo format)
Copy of your currently valid driver license. (showing expiration date)
